The Colorado State Innovation Model (CO SIM) strives to improve the health of Coloradans by providing access to integrated physical and behavioral health care services in coordinated systems, with value-based payment structures, for 80% of Colorado residents by 2019. Between 2015 and 2019, the State of Colorado will receive up to $65 million from the Center for Medicare and Medicaid Innovation to implement and test its State Health Care Innovation Plan, which focuses on providing access to integrated primary care and behavioral health services in coordinated community systems, applying value-based payment structures; expanding information technology efforts, including telehealth; and, finalizing a statewide plan to improve population health.
For SIM Cohort 2, HealthTeamWorks is formally collaborating with Colorado Children’s Healthcare Access Program (CCHAP) to provide in-depth transformation support for family medicine practices focusing on both adult and pediatric behavioral health integration and test alternative payment models. Please note that you do not need to be a pediatric practice to participate in work with us. We consider this collaboration a valuable and innovative opportunity for those SIM practices engaging in pediatric care to benefit from our hybrid of experience and expertise. We have extensive experience and capacity to work with adult-only practices.
