Evidence Now Southwest

EvidenceNOW Southwest served 260 primary care practices across Colorado and New Mexico with practice transformation and quality improvement support, including on-site practice facilitation and coaching, expert consultation, shared learning collaboratives, and EHR support. The initiative was aimed at transforming healthcare delivery by building critical infrastructure to help smaller primary care practices apply the latest medical research and tools to improve heart health.
