A Risk-Stratified Approach to Care Management Staffing

Eighty percent of the total healthcare spend in the US can be attributed to twenty percent of patients. These individuals are high need, high-risk and, without targeted care management support, will have exponential impact on cost & utilization, jeopardizing risk-based contracts.   

A tiered team approach to care management staffing enables each team member to work at top of license and allocates the appropriate resources to address the unique needs each patient and manage the risks associated.

In this roundtable event, we are exploring care management staffing models that effectively address these high-need, high-cost patients and rein in the threat to risk based contracts, all while meeting the needs of the entire population.
