The Transformating Clinicial Practice initiative (TCPi) prepares clinicians in Colorado to be successful with new models of compensation that require new models of care delivery, effective care coordination, and demonstrated value of care. The program focuses on business process development for new models of payment, change management and leadership skills development, efficient and effective referrals, reporting and using data to drive improvement and improve transitions of care.
