New Case Study: How HealthTeamWorks Helped Practices in Colorado Achieve Success in the Comprehensive Primary Care Plus Model

HealthTeamWorks has more than 25 years of experience supporting hundreds of practices. For 5 of those years, our team worked closely with practices participating in CMS’ Comprehensive Primary Care Plus (CPC+) Initiative as they moved from fee-for-service to population-based payment models.

This work proved to be a great success - promoting population health and chronic disease management, encouraging coordinated patient-centered care, and optimizing performance by providing actionable data and effectively capturing and diffusing new knowledge.

To further understand this success, we recently created a case study looking at how our facilitators worked with practices in Colorado to achieve success in the CPC+ model with the hopes of taking those learnings into future transformation efforts.
