Integration and coordination are the name of the game as we move into alternative payment models and regional accountability. A once siloed and fragmented healthcare system is acknowledging the importance of comprehensive coordinated care across settings, and in many cases forming clinically integrated networks (CIN). High performing networks and their ability to innovate and collaborate, greatly benefit patients and families.

While many practices remain independent, payment models that base bonuses and penalties on cost, quality, and other performance measures require a new level of interdependence in the care of an empaneled patient population. Many formal and informal models exist, ranging from affiliations to fully integrated delivery networks (IDN) with active community partnership. Regardless of design, accountable care of patients with complex needs and conditions requires an infrastructure of leadership and communication, data and technology, performance improvement methodology, care management and coordination, aligned incentives, and ready access to the right care at the right time and in the right settings.

  • How do current and future payment models inform strategic planning and investment pacing for a value-based delivery infrastructure?
  • How are decisions regarding investments in the network made (e.g. technology, staffing, services, etc.)?
  • How do you plan for, establish, train, and evaluate new roles and functions (e.g. care managers, coordinators, data analysts, practice facilitators, etc.)?
  • How do you manage change and foster a culture of continuous improvement in a learning organization?
  • How do you manage reporting demands and competing priorities?
