Colorado achieves Shared Savings in CPC for 2015!

This week, CMS announced the results of the national CPC program’s shared savings performance for 2015. Four of the seven regions participating in CPC – the states of Arkansas, Colorado, and Oregon, and the Greater Tulsa region in Oklahoma – realized net savings and will share in those savings with CMS.

HealthTeamworks^® is thrilled to announce that all of our Colorado practice clients that met the CPC reporting requirements also met the quality and utilization benchmarks and will jointly receive $450,086 in shared savings! Also of note, Colorado achieved the lowest all-cause hospital readmission rates of all CPC regions. We would like to thank all of our practices for their incredible work and continued participation in the CPC initiative. The results are a testament to the efforts they have made to deliver the best, most comprehensive primary care possible. We are encouraged by the results for every region as we further our commitment to support primary care.

As a group, all CPC practices showed positive quality results, with lower than expected hospital admission and readmission rates and favorable performance on patient experience measures. CPC practices’ performance on electronic Clinical Quality Measures also exceeded national benchmarks, particularly on preventive health measures. In a CMS blog post this week, Dr. Patrick Conway, CMS Deputy Administrator for Innovation and Quality and CMO said, “As the largest test of advanced primary care in U.S. history, CPC demonstrates the potential of primary care clinicians redesigning their practices to deliver better care to their patients, and provides clinicians support to innovate and deliver care in ways that better meet their patients’ needs and preferences…The positive performance is a testament to the efforts CPC practices have made to provide truly comprehensive primary care.”  We couldn’t agree more.
