Delaware State Innovation Model

The Medical Society of Delaware/MedNet has contracted with the State of Delaware, Department of Health and Social Services, Delaware Health Care commission (HCC) to support primary care practice transformation in Delaware. Primary care practice transformation is a key component of Delaware’s Health Innovation Plan, adding to the State’s goal of improving health care quality and experience, while reducing costs for all Delawareans. MSD/MedNet and HealthTeamWorks are committed to supporting Delaware physicians as they strive to enhance the health of Delaware’s communities.
