CPC+ includes all seven original CPC regions and integrates many lessons learned from CPC, including insights on practice readiness, the progression of care delivery redesign, actionable performance-based incentives, necessary health information technology, and claims data sharing with practices. CPC+ includes two primary care practice tracks with incrementally advanced care delivery requirements and payment options to meet the diverse needs of primary care practices in the United States (U.S.). Through a unique public-private partnership with 61 aligned payers in 18 regions (PDF), the CPC+ payment redesign gives practices the additional financial resources and flexibility they need to make investments that will improve quality of care and reduce the number of unnecessary services their patients receive. CPC+ provides practices with a robust learning system, as well as actionable data feedback, to guide their decision making. The care delivery redesign ensures practices in each track have the infrastructure to deliver better care to result in a healthier patient population.

 

HealthTeamWorks' Role

HealthTeamWorks provides regional support to 172 designated CPC Plus practices in eastern Colorado and 35 in Nebraska. 

HealthTeamWorks plays a role in the establishment and management of the National Learning Network component of the Learning System for CPC+. The National Learning Network functions across the entire initiative, engaging participating practices and payers as well as participating heath information technology (HIT) vendors in a common understanding of how Comprehensive Primary Care will operate to achieve the aims of better health, better care, and smarter spending. The National Learning Network is a component of the full Learning System for CPC+. The CPC+ Learning System is designed to optimize performance of the CPC+ model by providing actionable data and effectively capturing and diffusing new knowledge among participating practices, payers (including CMS), and HIT vendors and to fully engage participating practices, payers, and other stakeholders in achieving the CPC+ aims.
